For Your First Visit

What You Should Bring
  • On your first visit, please bring any X-rays, MRI, CT or myelogram films relating to your pain condition. Your referring physician's office should be able to assist you in obtaining these films and records.
  • A list of all medications you are taking, including the name, dosages (number of pills taken per day), milligrams and how long you have been taking the medication.
  • A list of medications you have taken within the past year, including name, dosages (number of pills taken per day), milligrams, how long you took the medication, its effectiveness and why you stopped taking it. Also, any allergic reactions you may have had are important to note.
Paying For Your Services

The Pain Center has professionals on staff who specialize in preauthorizing payment for services prior to your appointment. The staff works closely with Workers' Compensation, private insurance carriers, Medicare/Medicaid and most HMO plans.

Generally, the staff is able to obtain the necessary payment authorization from your insurance carrier and take that worry off your mind. However, in the event your claim is denied, you will be notified and someone at the Pain Center will work with you to make other payment arrangements.
